Keep medical records out of standard forms

Do not upload or paste medical records, imaging, laboratory reports, identification documents or detailed medical histories into standard website forms. They are intended for a brief, non-clinical overview. If information beyond that overview is needed, ask AN about an appropriate way to share it before sending anything.
